To UPDATE an Existing Record:
Enter your Email Address, First Name, Last Name. In addition, please add information into the fields to be changed.

To ADD a New Record for a Family Member:
Enter at least your Email Address, First Name, Last Name. We urge you to supply ALL requested information (see sample below), so we may better serve you. That way information can be sent to you by Email, Postal Service, Telephone, Text Messaging, or a combination of the above.
